The Triage RN plays a crucial role in delivering telephonic assistance to patients and their families. This position is responsible for fostering and maintaining positive relationships with team members, patients, families, and referral sources.

Qualifications:

  • Valid Florida or multi-state RN license, with the ability to apply for a multi-state license within 30 days of hire if not already held.
  • Capability to obtain a license to practice as an RN in Washington D.C. within 30 days of hire.
  • At least one (1) year of nursing experience; hospice or hospital experience is preferred.
  • Familiarity with EMR/EHR systems.
  • Proficient communication skills in English (both verbal and written); bilingual in Spanish is a plus.
  • Valid driver's license and automobile insurance in accordance with Company policy.
  • Reliable transportation to meet patient needs.

Job Responsibilities:

  • Manage after-hours phone calls, ensuring that callers' needs are met efficiently with excellent customer service.
  • Provide telephone advice and education tailored to patient needs.
  • Handle all telephone inquiries related to Chapters Health System's services in a timely, courteous, and effective manner, clearly explaining the benefits of our services.
  • Collect necessary information and documentation to ensure accurate patient records.
  • Accurately input data into tracking systems, including patient referral files in electronic medical records.
  • Offer reassurance to patients and families over the phone, assisting in finding solutions to their inquiries and coordinating in-person visits when necessary.
  • Help patients and families achieve desired comfort levels.
  • Utilize appropriate resources to resolve complex situations.
  • Document patient and family contact information in the EMR and communicate with the Interdisciplinary Group.
  • Report technical issues promptly and follow company protocols for resolution.
  • During emergencies, be prepared to report to work as designated by the company to ensure continuity of services.
  • Perform additional duties as assigned.
