Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a FT Weekend Baylor Licensed Practical Nurse. The successful candidate will be responsible for providing skilled nursing care to patients in their homes, under the direct supervision of a Registered Nurse (RN).

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with the interdisciplinary care team to ensure seamless care coordination and communication.
  • Document patient care activities and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current State LPN or LVN License
  • Graduate of an accredited practical nurse or vocational nursing program.
  • At least one year of nursing experience, preferably in community/home health or medical/surgical settings.
  • Valid BLS certification.
  • Excellent communication and interpersonal skills.
  • Licensed driver with a reliable vehicle and valid insurance.
