Job Summary

We are seeking an experienced RN-Case Manager to join our SSM Health float pool team. This individual will float to our 6 St. Louis locations, supporting our organization by ensuring patients are discharged from the hospital safely, identifying any barriers and working through those with the patient and care team.

Key Responsibilities
  • Ensures compliance with Condition of Participation (CoP) discharge planning.
  • Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
  • Assesses patients at high risk for readmission, identifies root cause for readmission, and develops a plan to collaborate with the multidisciplinary team regarding discharge planning to the next level of care.
  • Participates with other members of the team regarding opportunities for improvement in standard work.
  • May also be responsible for utilization management process, ED case management process, admission process, and acting as a case manager liaison with post-acute providers (skilled nursing facilities, rehab, home health, etc.).
Requirements
  • Graduate of an accredited school of nursing or education equivalency for licensing.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of work location.
Work Environment

This role will work in a fast-paced healthcare environment, requiring frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. The individual will also use vision and depth perception for distances near and far, and to identify and distinguish colors. The role will require frequent keyboard use/data entry and occasional bending, stooping, kneeling, squatting, twisting, and gripping.
