About the Role:

Nurse Practitioner - In-Home Care

As a Nurse Practitioner at EasyHealth, you will be at the forefront of delivering preventive care and educational services, directly engaging with members in the heart of their communities.

This position offers the unique fulfillment of local travel and the opportunity of face-to-face interactions with members directly in the comfort of their homes.

You'll play a vital role in performing comprehensive assessments such as Annual Wellness & Quality Gap Care visits for our members in their homes.

The applicants must be local and have reliable transportation for travel to at least one of these areas.

Key Responsibilities:

  • Perform clinical screenings and various tests in an in-home setting.
  • Accurately code, verify, and document clinical findings.
  • Complete a detailed evaluation of the medical and social assessment of the beneficiary members using the company's proprietary tool or client EMR.
  • Identify and refer members/patients for routine, urgent, and emergent case management.
  • Make recommendations for member's follow-up with their primary care physician.
  • Maintain a comprehensive understanding related to protected health information and observe HIPAA requirements.
  • Follow the safety policy and procedures listed subsequently.

Requirements:

  • Board-certified, preferably geriatric or adult medicine specialty.
  • Familiarity with the HCC model and quality HEDIS and other STAR measures.
  • Proficient in the use of computer, tablet, and mobile technology.
  • Willingness to travel within the geographic area.
