Job Title: Psychiatric Mental Health Nurse Practitioner

Job Summary:

InnovAge is seeking a highly skilled Psychiatric Mental Health Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide high-quality, clinically appropriate behavioral health care to our participants. Responsibilities:
  • Develop and deliver personalized care plans for participants, ensuring their unique needs are met.
  • Provide in-person clinical assessments, prescribe and deprescribe medications, and manage ongoing medication therapy.
  • Coordinate care with behavioral health providers, primary care physicians, and interdisciplinary team members.
  • Consult with team members on behavioral interventions, nursing practice, and psychopharmacology.
  • Monitor psychiatric hospitalization care and follow-up disposition planning.
  • Respond to urgent calls from clinics, center staff, and facilities with a solution-focused approach.
  • Monitor response to psychotropic medications.
Requirements:
  • Current State-issued Psychiatric Nurse Practitioner/PMHNP license.
  • 2+ years of experience in psychiatric care and medication management.
  • 5+ years of nursing experience, with at least 2 years as a Nurse Practitioner.
  • DEA Certification with full prescriptive authority.
  • Proficiency with standard Microsoft Office applications and EMR systems.
  • Current CPR certification or ability to obtain within 30 days of employment.
Preferred Qualifications:
  • Experience working with geropsychiatric populations.
  • Licensure or certification in substance use treatment.
  • Training or experience with group and family interventions.
  • Experience with conflict management, mediation, de-escalation, and crisis mitigation.
  • Knowledge of chronic disease management and psycho-social implications.
  • Bilingual.
