About the Role

We are seeking a skilled RN Case Manager to join our team in Downtown Atlanta, GA. As a hybrid remote role, you will have the opportunity to work from home and travel within an assigned geography for in-person visits.

Key Responsibilities
  • Support complex patients in navigating the healthcare system through medical record review and consultation.
  • Identify and address medical, social, emotional, and financial needs of patients with CKD/ ESKD.
  • Work autonomously and collaboratively with the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.
Requirements
  • Current RN License is required, BSN preferred.
  • Minimum of 2 years RN experience required with healthcare performance coaching.
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required.
