Job Summary

We are seeking a highly skilled RN Admissions Case Manager to join our team at Ochsner Health System. As a key member of our healthcare team, you will be responsible for coordinating patient admissions and transfers, ensuring seamless transitions of care, and promoting optimal patient outcomes.

Key Responsibilities
  • Direct and coordinate all admissions and transfers to, from, and within the hospital, collaborating with interdisciplinary teams to ensure efficient patient flow.
  • Facilitate professional relationships between unit leadership, operational coordinators, nursing staff, and other stakeholders to provide patient/room assignment based on patient diagnosis, acuity, and MD orders.
  • Manage aspects of the planning process in anticipation of the patient's movement through the healthcare system, ensuring timely and effective care transitions.
  • Interact with managed care/third-party payers to obtain financial certification for appropriate reimbursement and minimize reimbursement denials.
  • Manage the care of assigned patients through the healthcare system, serving as a contact person for the rehabilitation team, internal admissions team, clients, and external sources to enhance outcomes.
  • Educate clients, family members, referral sources, hospital personnel, external payers, and internal/external case managers about rehabilitation program benefits and services.
Requirements
  • Registered nurse diploma required; Associate's degree in nursing preferred.
  • 1 year of hospital-based experience in the delivery of patient care required; experience in case management or utilization review preferred.
  • Current Registered Nurse license in state of practice required; Commission for Case Manager (CCM), Certified Professional Utilization Review (CPUR), Certified Professional in Utilization Management (CPUM), or Certified Professional in Healthcare Management (CPHM) certification preferred.
