RN Case Manager

Ochsner Health System is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring that patients receive the necessary care and resources to achieve optimal health outcomes.
Key Responsibilities- Perform pre-admission, admission, and continued stay reviews to ensure medical necessity and appropriateness of care
- Collaborate with the care team to develop and implement discharge plans that meet the individual needs of patients
- Communicate effectively with patients, caregivers, and healthcare providers to ensure seamless transitions of care
- Review and recommend alternative care options to optimize patient outcomes
- Maintain accurate and up-to-date documentation of patient information and care plans
- Current registered nurse license in the state of practice
- Basic Life Support (BLS) certification from the American Heart Association
- 3+ years of hospital-based experience in discharge planning, case management, or utilization review
- Preferred: Certification in Case Management (CCM) or Accredited Case Manager (ACM)
