At Methodist Hospital Metropolitan, we are committed to delivering exceptional patient-centered care. As a Registered Nurse for Admission and Discharge, you will play a vital role in ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings.
Responsibilities:- Obtain a nursing assessment history and ensure proper inpatient care unit admission.
- Take appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
- Complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
- Teach patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures.
- Translate complex medical terminology to ensure complete knowledge.
- Complete extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
- Ensure patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
- Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
- Registered Nurse Diploma, or Associate Degree, or Bachelors Degree
- 1-2 years Admission Discharge Lounge experience or Medical Surgical/Tele experiences

Our women's pavilion is the only freestanding, center city facility dedicated to the health of women and their newborn babies.
