Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to our Medicare patients.

Job Summary:

The Nurse Practitioner will be responsible for providing primary care services to patients, including conducting office visits, administering wellness visits and health risk assessments, and coordinating care with other healthcare providers. The ideal candidate will have a strong passion for delivering exceptional patient care and a commitment to our mission of rebuilding healthcare as it should be.

Responsibilities:
  • Provide primary care services to patients, including conducting office visits and administering wellness visits and health risk assessments.
  • Coordinate care with other healthcare providers, including specialists and testing facilities.
  • Participate in population health leadership, including making sure all eligible females receive evidence-based breast cancer screening every 2 years.
  • Educate patients on their health conditions, care plans, and treatments.
  • Participate in Oak Street Health promotional activities.
  • Conduct home visits as needed.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification as a Nurse Practitioner through AANP or ANCC.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.
Preferred Qualifications:
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration.
  • Desire to be part of an innovative model focused on empirically-guided population health.
