Job Summary

The Team Registered Nurse is a key member of our interdisciplinary team, responsible for assessing and identifying patient/family needs, and planning, implementing, evaluating, and documenting individualized hospice nursing care. This role requires collaboration with other medical providers and supervising the care provided by Licensed Practical Nurses and Certified Nursing Assistants.

Responsibilities
  • Assess and identify patient/family needs, and develop individualized care plans in accordance with the interdisciplinary plan of care and organizational policies.
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Collaborate with the Interdisciplinary Group and other medical providers to ensure comprehensive care.
  • Supervise the care provided by Licensed Practical Nurses and Certified Nursing Assistants.
  • Document patient care and progress in the electronic medical record.
Requirements
  • RN with current licensure in the state of Kentucky and/or Indiana, or the ability to obtain required licenses upon offer of position.
  • BSN desired.
  • Certification in hospice/palliative care preferred.
  • One year of nursing experience required.
  • Knowledge of palliative care helpful.
  • Valid driver's license required.
  • Proficiency in technology-based tools, including electronic medical records.
