The Registered Nurse (RN) will undertake the following responsibilities for A&C Private Home Care (the "Scope of Work"):
a. The RN will oversee the personal care, companionship, and sitter services provided by A&C Private Home Care. The RN will be the main supervisor for personal care tasks for clients identified as medically vulnerable or compromised. The RN shall ensure that personal care tasks are delivered in accordance with established standards of care.
b. The RN will evaluate both physical and mental health requirements, monitor vital signs as necessary, observe the functional capabilities of clients, and document any changes in the client's health status in their records. The RN will educate and motivate clients towards self-care, coordinate care plans with physicians and other service providers, and supervise the creation and execution of the care plan. Additionally, the RN will oversee personal care tasks for clients identified as medically vulnerable or compromised.

Core Responsibilities Include:
• Client assessments
• Execution of service plans
• Preventive and rehabilitative nursing practices
• Preparation of clinical progress notes
• Collaboration with other service providers
• Informing physicians and case managers of any changes in the client's condition or additional requirements
• Providing health-related education tailored to client needs
• Supervising and training other staff members
• Coordinating specialized therapies and overseeing services for medically vulnerable clients.
The work will be completed upon review and approval by A&C Private Home Care, which approval shall not be unreasonably withheld.
Change Orders:
If A&C Private Home Care makes any modifications to the Scope of Work, it must be documented through a written "Change Order" signed by both parties, detailing the additional work to be performed and any associated costs.
Compensation:
A&C Private Home Care will compensate the Contractor as follows:
• A fee of $____________50_________________ per visit
• A mileage reimbursement rate of $.31 per mile. Both the fee and mileage will be paid in the subsequent payroll period upon completion of the work.
The RN will document all activities and submit the documentation along with an invoice to A&C Private Home Care for payment.
Independent Contractor Relationship:
Unless otherwise mandated by law, the RN is considered an independent contractor, not an employee of A&C Private Home Care, and is solely responsible for all taxes incurred in connection with this agreement. A&C Private Home Care will not withhold FICA (Social Security and Medicare taxes) or state or federal income tax on behalf of the RN, nor will it make state or federal unemployment contributions unless required by law.
Permits, Certificates & Licenses:
The RN is responsible for determining any necessary Georgia state and local permits, certificates, and/or licenses required for undertaking and completing the work, and for obtaining those documents. Copies of all permits, certificates, and/or licenses must be provided to A&C Private Home Care for record-keeping purposes.
Insurance:
The RN will maintain adequate insurance to cover any loss or injury resulting from the actions of the RN, its employees, or subcontractors in connection with this agreement.
LIMITED WARRANTY:
The RN warrants that the work performed under this agreement will be completed in a timely and professional manner, meeting or exceeding the standards for similar work in the community or region, and will comply with all applicable building codes and requirements.
