Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Nurse Specialist. As a key member of our team, you will be responsible for providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Key Responsibilities

  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop an individualized plan of care within state-specific guidelines.
  • Coordinate total plan of care with other healthcare professionals involved in care and ensure continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.
  • Perform patient comprehensive assessments at designated time points and develop the appropriate plan of care to ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
  • Collaborate with physicians to ensure accurate, complete, and timely documentation of patient care and services provided.
  • Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure seamless care coordination and patient satisfaction.

Requirements

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
