Job Summary:
Home Health Care 2000 is seeking a highly skilled Psychiatric Registered Nurse to join our team as a Home Health Specialist. As a key member of our interdisciplinary team, you will provide one-on-one, psychiatric/behavioral health and skilled nursing care and treatment to patients in their home.
Key Responsibilities:
- Evaluate and assess the medical, cognitive, emotional, psychosocial, safety, developmental status, and behavioral health/psychiatric nursing needs of the patient to determine the appropriateness of care and criteria for acceptance to behavioral health/psychiatric home health care.
- Collaborate with mental health professionals, including psychiatrists and psychologists, to create a comprehensive home care plan supporting individuals and their caregivers coping with dementia, Alzheimer's, schizophrenia, anxiety, grief, depression, and other psychiatric conditions within the home setting.
- Act as a case manager in coordinating care for the behavioral health/psychiatric patient, regularly reviewing treatment planning under the direction of a physician or psychiatrist, which includes nursing actions and interventions, goals, and discharge summaries.
- Administer IM medications, perform lab draws and specimen collections, and educate patients and caregivers in appropriate medical techniques, performing other nursing treatments in accordance with the plan of care.
Requirements:
- 3 years of experience in a mental health outpatient clinic
- 1 year of recent nursing experience in an active treatment in a psychiatric unit
- 3 years of experience in mental health in a hospital setting
- 3 years of experience in a clinical setting
- Bachelor's degree in Nursing
- Registered Nurse (RN) State of Louisiana licensure
