About the Role

The Patient Care Manager will be responsible for overseeing patient care activities, ensuring compliance with regulatory requirements, and promoting quality care and safety standards. As a leader, you will motivate and inspire staff to provide exceptional care, liaise between staff, patients, and the medical community, and lead by example.

Responsibilities
  • Lead, inspire and motivate others to provide exceptional care.
  • Evaluate patient care needs and develop individualized plans to ensure safe and efficient care delivery.
  • Liaise between staff, patients, and the medical community to ensure effective communication and coordination.
  • Monitor and improve quality of care and patient safety through ongoing evaluation and assessment.
  • Develop and implement policies and procedures to ensure compliance with local, state, and federal regulations.
Requirements
  • Current RN licensure in the state of practice.
  • Minimum 4 years of clinical experience as an RN, preferably in hospice care.
  • Current CPR certification.
  • A reliable vehicle and valid driver's license (or access to public transportation).
