About the Role:
Sunrise Senior Living is seeking a skilled LPN Medication Care Manager to join our team. As a key member of our care team, you will be responsible for administering medication and treatments in a safe and organized manner.
Responsibilities:
- Administer medication and treatments to residents in accordance with their individual care plans
- Monitor residents' responses to medication and treatments, and report any concerns to the care team
- Collaborate with the care team to develop and implement individualized care plans
- Provide education and support to residents and their families on medication management and treatment options
Requirements:
- Current LPN licensure in the state of employment
- Experience in long-term care or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
