We are seeking a compassionate RN Case Manager to join our team of professionals who provide hospice/palliative care in Riverside County.

Job Summary

The registered nurse plans, organizes, and directs hospice care, with a focus on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions
  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Provide physical assessments and histories of current and previous illnesses.
  • Develop and implement care plans that establish goals based on nursing diagnoses and incorporate palliative nursing actions.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel and provide healthcare instructions to patients as appropriate.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and optional comfort care.
  • Manage assigned caseload by supervisor.
  • Document accurate and relevant clinical notes regarding patients' conditions and care given.
  • Communicate with physicians regarding patients' needs and report changes in patients' conditions.
  • Attend interdisciplinary group meetings.
Position Qualifications
  • Current RN license to practice professional nursing in the state of CA.
  • Graduate of a National League for Nursing accredited school of nursing.
  • Current CPR certification.
  • Licensed driver with an automobile that is insured.
  • A minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license.
  • Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing.
  • Demonstrates excellent observation, problem-solving, verbal, and written communication skills.
  • Ability to organize, prioritize workload independently, and document timely.
