Job Summary:
The primary emphasis is to partner with all other members of the healthcare team to ensure efficient care of inpatients, avoiding delays and denials whenever possible.
As a Social Work Case Manager, you will take a lead role in coordinating with community agencies to address abuse, neglect, substance abuse, mental health, or adoption, linking to financial resources, and providing support to patients and their families.
Key Responsibilities:
- Partner with healthcare teams to ensure efficient care of inpatients
- Coordinate with community agencies to address various patient needs
- Link patients to financial resources and provide support
- Collaborate with multidisciplinary teams to develop and implement patient care plans
Requirements:
- Master's degree in Social Work required
- 2-4 years of Social Work experience required
- Recent Social Work experience in Medical Social Work is preferred
- LCSW-C - Certified Clinical Social Worker required
