Job Summary

The Occupational Therapist will be responsible for conducting comprehensive assessments, developing and implementing treatment plans, and collaborating with the interdisciplinary team to ensure optimal patient outcomes.

Key Responsibilities
  • Conduct initial and periodic assessments of Eldercare participants to identify their needs and develop individualized treatment plans.
  • Collaborate with the interdisciplinary team to develop and implement care plans that promote patient independence and safety.
  • Participate in interdisciplinary team meetings to discuss patient progress and make recommendations for care planning decisions.
  • Develop and implement individual and group activities to promote patient participation and engagement.
  • Assess the home environment for safety and develop plans to modify it to support patient independence and safety.
  • Supervise and direct Occupational Therapy Assistants to ensure they are providing high-quality care to patients.
  • Provide education and training to patients and their families on proper care and use of durable medical equipment.
Requirements
  • Current Florida State OT License.
  • BLS or CPR Certification.
  • Graduate of an accredited Occupational Therapy Program.
  • A minimum of 3 years of clinical work experience in a specific setting and a minimum of 1-year experience working with a frail or elderly population.
