We are seeking a skilled and compassionate RN Case Manager to join our team at Geisinger Hospice. As a Hospice RN, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.

Key Responsibilities:

  • Conduct thorough nursing evaluations to determine patient eligibility for hospice services.
  • Develop and implement personalized care plans that address the physical, psychosocial, and environmental needs of our patients.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on best practices in hospice care.

Requirements:

  • Current RN licensure in the state of practice.
  • One year of clinical experience in a healthcare setting.
  • Current CPR certification.
  • Valid driver's license and access to a dependable vehicle.
