As a Care Coordinator RN Per Diem, you will play a critical role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. Your responsibilities will include:

  • Providing program orientation to patients, families, and caregivers, covering the case manager's role, rehabilitation philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
  • Acting as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status through Family Conferences and other interactions.
  • Ensuring the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process.
  • Documenting findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.

This position requires an RN with current state licensure and three years' clinical acute care experience. A Bachelor's degree in Nursing is preferred. Certification in case management or utilization review is also beneficial. InterQual experience is a plus.

The estimated salary for this role is $62,000 - $82,000 per year, depending on experience.
