Transplant Center Overview

About the Role


We are seeking a skilled Critical Care Coordinator to join our team. As a Critical Care Coordinator, you will be responsible for coordinating pre- and post-transplant patient activities, ensuring seamless transitions between outpatient and inpatient care.



Key Responsibilities:



  • Assess patients in the outpatient clinic alongside transplant physicians to develop personalized care plans.
  • Facilitate effective communication with outpatient caregivers to ensure optimal symptom management.
  • Coordinate scheduling for follow-up appointments, streamlining the care process.
  • Review medical information from referring physicians and initiate necessary communication to ensure accurate patient data.
  • Collaborate with other disciplines to obtain consultation and follow-up information, enhancing patient care.
  • Coordinate with hospital unit personnel for efficient discharge of transplant patients and donors.
  • Obtain medications and supplies as needed, adhering to patient insurance coverage.
  • Contact referring physicians to arrange follow-up appointments, ensuring continuity of care.
  • Coordinate pre-transplant evaluation with transplant physicians and referring physicians.
  • Assemble test results and schedule additional testing as required.
  • Presentation cases to the Selection Committee for thorough review.
  • Place patients on the national waiting list for transplantation.
  • Provide ongoing care for inpatients and outpatients awaiting transplant.
  • Triage patients during the transplant process for medical and psychosocial issues.
  • Collaborate with Office Coordinators and Financial Counselors for insurance clearance.
  • Distribute informational packets and conduct educational sessions for patients and families.
  • Work closely with the Organ Procurement Organization for organ procurement and transplant surgery.


Requirements


We require a minimum of two years RN experience, including one year acute care hospital experience, with disease-related experience. Basic Life Support (BLS) Certification is mandatory. Preferred qualifications include at least two years RN experience working in ICU or ED, a Bachelor's Degree, and the ability to multitask and prioritize.



Work Environment


This role demands working under stressful conditions and irregular hours. The caregiver must be willing to be on call 24/7 on a rotating basis.



Estimated Salary Range: $80,000 - $110,000 per annum
