About the Role

As a Home Health RN Coordinator, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. You will work closely with our multidisciplinary team to develop and manage patient care plans, ensuring seamless transitions from inpatient to home care settings.

Responsibilities
  • Provide direct patient care, including assessments, interventions, and education
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and implement patient care plans
  • Manage patient caseloads, ensuring timely and effective care delivery
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • Current RN license in the State of Idaho
  • 2 years of RN experience, with hospice or acute care experience preferred
  • Basic Life Support (BLS) certification
  • Valid driver's license and proof of auto insurance
