About the Role

VITAS Healthcare is seeking a skilled Registered Nurse to join our interdisciplinary team. As a key member of our hospice care team, you will provide high-quality, patient-centered care to individuals and their families facing terminal illness and bereavement.

Key Responsibilities
  • Coordinate patient care and ensure continuous assessment of each patient's and family's needs
  • Implement the interdisciplinary plan of care and provide education and training to patients, families, caregivers, and referral sources
  • Participate in patient care meetings and provide effective communication and shift reports
  • Provide visits and shifts in program coverage areas and work with the IDG to develop, coordinate, and follow the patient's individualized plan of care
  • Delegate nursing tasks to Hospice Aides, Volunteers, and Licensed Practical/Vocational Nurses
  • Participate in the Quality Assessment and Performance Improvement (QAPI) Program and related activities
  • Represent the nursing profession, the VITAS site/location, and VITAS overall in a positive manner
Requirements
  • Currently licensed to practice nursing in the state where the VITAS program is located
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
  • Ability to stand or walk in excess of one hour per day
