Job Summary
">As a Registered Physical Therapist (RPT), you will play a pivotal role in helping individuals achieve their highest level of motor functioning and mobility. You will collaborate with consumers to develop tailored therapy goals and provide supervision to physical therapy assistants, ensuring seamless support throughout the rehabilitation process.
Responsibilities and Duties
- Leveraging Expertise: Embody Minds Matter, LLC's philosophical principles in all aspects of service delivery, demonstrating a deep understanding of our values and mission.
- Leadership Role: Review, co-sign, and provide constructive feedback on daily notes from assistant therapists, fostering a culture of collaboration and open communication.
- Empathetic Approach: Assist individuals in identifying their needs and developing effective compensation techniques to adapt to the consequences of their brain injury, offering creative solutions and alternatives for everyday activities.
- Evaluation and Analysis: Conduct thorough evaluations addressing consumers' physical and mental abilities, analyzing medical data to determine the most effective therapies and strategies.
- Documentation and Reporting: Maintain accurate documentation as required by individual plans of care, including evaluations, progress notes, daily notes, letters of medical necessity, and other necessary records.
- Collaborative Environment: Integrate employment, educational, and independent living goals into therapy, working closely with consumers and professional staff to ensure seamless goal attainment.
- Team Collaboration: Participate in team meetings, assist consumers in goal setting, and contribute to a positive, supportive team dynamic.
- Service Delivery: Provide services only as designated on each individual consumer's authorized plan of care, upholding the highest standards of professionalism and expertise.
- Assessment and Recommendations: Assess and recommend adaptive equipment and/or home modifications, as well as processes for home and community environments, to enhance consumers' quality of life.
- Personalized Support: Work directly with consumers to improve skills in all areas of their life goals, providing goal-driven, concise, factual, and clear documentation of sessions.
- Family and Caregiver Education: Educate and train families and caregivers about the consequences of brain injury strategies and techniques, maintaining open communication about goals and progress.
- Professional Development: Attend all trainings and workshops to maintain the latest evidence-based practices, staying abreast of industry advancements and best practices.
- Rapport Building: Establish and maintain professional relationships with consumers, co-workers, referral agents, and representatives of other community agencies and services.
- Public Relations: Promote public relations throughout the agency and community for the program, contributing to a positive image and reputation for Minds Matter, LLC.
- Task Groups: Demonstrate willingness to serve on agency and interagency task groups as requested, showcasing adaptability and a commitment to teamwork.
- Other Responsibilities: Perform other duties as assigned by supervisors, including care and maintenance of Minds Matter, LLC-issued iPad and iPhone.
- Confidentiality: Maintain confidentiality of protected health information in accordance with HIPAA regulations, upholding the highest standards of professionalism and ethics.
- Team Lead Role: Possibly serve as a consumer's team lead, overseeing updating of goal plans and team meeting history forms during team meetings, tracking progress, effectiveness of strategies, concerns, task assignments to team members, or changes to goals.

- Tutorial Expertise: Ability to teach skills to individuals with disabilities, utilizing your expertise to empower and educate.
- Leadership and Mentorship: Effectively supervise and mentor therapy assistants, providing timely suggestions, clear feedback, and guidance as needed, fostering a culture of growth and development.
- Sensitivity and Empathy: Possess a high degree of sensitivity towards all people, recognizing the unique challenges and strengths of each individual.
- Interpersonal Skills: Ability to deal professionally with persons on a one-to-one basis, building strong relationships and trust.
- Care Coordination: Ability to effectively plan, coordinate, and assist with all aspects of a consumer's life, prioritizing their needs and goals.
- Time Management: Excellent time management skills, often changing from one task to another without loss of efficiency or composure, adapting to the demands of a dynamic environment.
- Creativity: Demonstrate creativity in developing innovative solutions and alternatives for consumers, thinking outside the box to meet their unique needs.
- Communication: Excellent verbal and written communication skills, facilitating seamless collaboration and open communication among team members and stakeholders.
- Deadline Management: Ability to meet deadlines, prioritizing tasks and managing time effectively to ensure successful project completion.
- Technology Proficiency: Basic to moderate technology skills, comfortable using electronic medical record systems and navigating digital tools with ease.
Education and Experience
- Academic Excellence: Successful completion of a physical therapy program from an accredited university, coupled with current licensure in the state of Kansas.

- Verbal and Listening Skills: Requires good verbal skills and listening ability, enabling effective communication and empathy.
- Reading and Writing: Must be able to read and write, possessing basic literacy skills and the ability to document accurately.
- iPad Proficiency: Basic iPad proficiency, comfortable using digital tools and navigating electronic medical records.
- Reliable Transportation: Have reliable transportation, ensuring timely arrival at appointments and visits.
- Communication Check-In: Check voicemail and email daily, maintaining open lines of communication and staying connected with team members and stakeholders.
