Job Title: SSVF Case Manager

Join The Salvation Army Central Territory as a SSVF Case Manager and make a meaningful impact in the lives of Veterans and their families. As a key member of our team, you will provide supportive services using the critical time intervention strength-based case management approach to help individuals break the cycle of crisis and vulnerability in their lives.

Job Summary:

As a SSVF Case Manager, you will work closely with Veterans and their families to identify and address their unique needs, providing linkages to community services and resources. Your goal will be to help them achieve sustainable permanent housing interventions and improve their overall well-being.

Responsibilities:
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional communication practices.
  • Develop and implement case plans with participants, including short- and long-term goals.
  • Provide regular meetings with participants to review and update goals, conduct home visits as needed, and offer linkages to community services.
  • Implement educational activities, such as life skills and budgeting, as needed.
  • Provide referrals for Veterans/families ineligible for SSVF services.
  • Maintain comprehensive demographic data and submit monthly summary of service statistics to direct supervisor and/or SSVF Program Manager.
  • Participate in quality assurance as requested by the SSVF Program Manager.
Requirements:
  • Bachelor's degree in social work, public health, and/or human services, or previous experience working with the homeless population and/or working with Veterans and in-depth knowledge of VA programs and guidelines.
  • Ability to efficiently and effectively manage people and projects successfully.
  • Team player, self-starter, and perform well with minimal supervision.
  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, Internet, and Social Media platforms.
Work Environment:

This Position is required to do basic business and office work. In order to successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit, and use hands. Individual must be able to regularly lift up to 30 pounds and occasionally lift up to fifty pounds. The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position.
