Evergreen Commons Rehabilitation and Nursing Center is seeking a skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As a Weekend LPN, you will provide nursing care to residents in our sub-acute rehab and skilled nursing facility.

Responsibilities:

• Collecting information from residents to be admitted, including medical records and insurance details.
• Recording health details of residents, including vitals and temperature.
• Administering medications and injections to residents as needed.
• Treating and dressing wounds and bedsores as needed.
• Supervising Certified Nursing Assistants (CNAs) as required.
• Assisting residents with personal hygiene and daily living activities.
• Monitoring residents' food and liquid intake and output.

Requirements:

• Valid LPN License
• Up-to-date MMR vaccination and physical
• Good standing with the State Registry
