Nurse Care Coordinator

As a skilled Nurse Care Coordinator, you will play a vital role in delivering exceptional patient care at Tri-State Hospital. Your primary responsibility will be to assess patients' physical, psychosocial, and behavioral health needs, advocating for their well-being and assisting healthcare providers in providing comprehensive care. You will also be responsible for documenting patient information in the Electronic Medical Record (EMR) according to Tri-State Hospital's policies and procedures.
Key responsibilities include:
- Assessing patients' health needs and developing individualized care plans
- Collaborating with healthcare providers to ensure seamless patient care
- Documenting patient information in the EMR
- Providing education and support to patients and their families
Requirements:
- Associate's degree or Bachelor's degree in healthcare administration or a related field
- Current Washington State RN license
- American Heart Association's BLS/CPR certification
- Excellent communication and interpersonal skills
