Job Opportunity

Heritage Home Health and Hospice is seeking a dedicated and compassionate Registered Nurse to join our team. As a community leader in home-based care, we provide individualized care tailored to meet the needs of people of all ages, helping them stay at home.

Responsibilities

The successful candidate will work with patients, their families, and other healthcare professionals in the patient's place of residence. Key responsibilities include:

  • Performing visits and completing coordination of care
  • Being the point of contact for all disciplines involved with providing care to patients
  • Following agency, state, and federal policies and procedures
  • Consulting with the patient's primary care physician, collecting OASIS information, and ensuring medication profiles are accurate
  • Attending meetings as required

Requirements

To be considered for this role, applicants must:

  • Be currently licensed as a Registered Nurse (RN) in the state of Idaho
  • Have a minimum of one year of clinical experience (preferred but not required)
  • Demonstrate knowledge and skill in nursing practice
  • Hold CPR professional certification
  • Have general knowledge of computers and the ability to learn industry-specific software and documentation applications
  • Possess a valid state driver's license and automobile liability insurance
