About the Role

We are seeking a skilled Nurse Practitioner to join our team as a Home Health Specialist. In this role, you will conduct in-home health assessments on enrolled Medicare Advantage members, providing comprehensive care and support to help them achieve optimal health outcomes.

Key Responsibilities
  • Conduct in-home assessments, averaging 45-60 minutes in length, to gather information on patients' medical history, review of systems, medication, and evidence-based practice screenings.
  • Check vitals, conduct physical exams, and perform point-of-care testing as needed.
  • Identify diagnoses and formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Identify needs and opportunities for clinical referrals and intervene appropriately in urgent and emergent situations.
  • Educate members on disease processes, medication, and compliance.
Requirements
  • NP license and national certification (ANCC or AANP) or the ability to obtain national certification and/or NP license in the state of assignment.
  • Family, Geriatrics, Adult, Adult-Gerontology Primary, or Adult-Gerontology Acute Care certification.
  • Access to reliable transportation to travel to client and/or patient sites within a designated area.
Preferred Qualifications
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
