Job Summary:

Care Partners is seeking a highly skilled and experienced PACE Site Director to oversee our PACE program, ensuring that our patients receive the best possible care. As a PACE Site Director, you will play a pivotal role in shaping the future of healthcare delivery in the Central Valley.

Key Responsibilities:

  • Lead the Delivery of Care and Clinical Outcomes: Oversee the delivery of high-quality, patient-centered care to the elderly and long-term care population.
  • Provide Medical Supervision and Leadership: Guide and mentor our medical team to reach their full potential, including nurse practitioners, physician assistants, and new MDs/DOs.
  • Oversee Quality Assurance and Performance Improvement Plans: Ensure that our patients receive the best possible care by developing and implementing quality assurance and performance improvement plans.
  • Mentor and Develop Team Members: Mentor and develop our team members to ensure that they have the skills and knowledge needed to provide high-quality care.

What You Bring:

  • Education & Licensure: A current, unencumbered California MD or DO license, and board certified or board eligible in a Primary Care specialty.
  • Experience: Minimum of three years in a lead administrative role, and at least one year of primary care experience with a frail, elderly, or long-term care population.
  • Managed Care Expertise: At least two years of experience working in a managed care environment, with a background in resolving issues related to utilization, quality management, performance improvement, and physician leadership.
  • Certifications: A valid BLS CPR certification from the American Heart Association.
  • Skills & Abilities:
    • Excellent Interpersonal Communication Skills: Excellent interpersonal communication skills.
    • Ability to Work Sensitively and Effectively with Individuals from Diverse Ethnic and Cultural Backgrounds: Ability to work sensitively and effectively with individuals from diverse ethnic and cultural backgrounds.
    • Strong Time Management Skills and the Ability to Lead in a Fast-Paced Environment: Strong time management skills and the ability to lead in a fast-paced environment.
    • Ability to Handle Confidential Information with Integrity: Ability to handle confidential information with integrity.
    • Valid California Driver's License, Acceptable Driving Record, and Vehicle Insurance: Valid California Driver's License, acceptable driving record, and vehicle insurance to travel to various sites as needed.

Physical Demands:

This role involves lifting and moving patients when necessary, so you should be comfortable with occasional physical exertion. You must also have good manual dexterity, hearing, and vision to effectively communicate and perform clinical tasks.

This role is hands-on and requires a compassionate approach to care. While the work environment can be fast-paced and occasionally intense, you'll be surrounded by a supportive team that's just as committed to making a difference as you are.
