Are you a registered nurse with a passion for providing compassionate care to patients in their final stages of life? This position as a Hospice Nurse is essential in delivering comfort and assistance to patients and their families during this profound journey.
Position Summary:
- Collaborate with a committed team to deliver exceptional end-of-life care.
- Engage in meaningful communication with patients and their families, offering emotional support and guidance.
- Honor and celebrate the individual life stories of each patient entrusted to your care.
Work Schedule:
Shifts from 9am to 7:30pm, including weekend rotations to provide comprehensive care coverage.
Primary Duties:
- Perform comprehensive assessments to identify hospice service requirements.
- Inform patients and families about available care options and support services.
- Work alongside physicians and healthcare professionals to create tailored care plans.
- Enhance community awareness of hospice care through educational outreach initiatives.
Required Qualifications:
- Valid RN license in Florida; preference for candidates with a Bachelor of Science in Nursing (B.S.N.).
- Experience in medical-surgical environments is advantageous.
- Certification in Community Public Health Nursing (CPHN) is beneficial.
- Proven skills in patient assessment, effective communication, and organizational abilities.
- Familiarity with electronic medical record systems and multilingual proficiency are considered assets.
- Ability to work independently while upholding high standards of patient care.
