We are seeking a compassionate and skilled RN Case Manager to join our Hospice team at Trustbridge. As a Hospice Nurse Case Manager, you will play a vital role in providing end-of-life care to patients and their families.

Responsibilities
  • Coordinate patient care under the direction of the primary physician
  • Conduct assessments of patient and family needs
  • Develop and implement individualized care plans
  • Ensure timely and effective implementation of interventions and services
  • Prioritize patient and family comfort and satisfaction
Requirements
  • RN degree from an accredited college or university
  • Current RN licensure in the State of Florida
  • BSN preferred
  • Hospice certification preferred
  • Basic Life Support Certification
Preferred Qualifications
  • One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing
  • Understanding of Hospice Nursing principles and practices
  • Basic computer skills and EMR software experience
