Job Description
We are seeking a skilled Per Diem Licensed Social Worker to join our team at Wind Crest by Erickson Senior Living. In this role, you will provide social work services to residents and family members of our Continuing Care community.
Key Responsibilities
- Complete admission assessments, MDS, and other required documentation in accordance with State and Federal regulations.
- Educate residents/families on the importance of advance directives and facilitate completion, signature, and tracking of documents.
- Coordinate the Social Work function from admission to discharge, including room changes and transition planning.
- Provide psychosocial support to residents and families to ensure a successful transition to Continuing Care.
- Participate in care/service plan meetings with family members.
- Provide bereavement support to residents, family members, and staff.
Requirements
- Social Work degree (BSW or MSW) required, or minimum education requirements as per state licensure.
- Minimum of 3 years' experience in working with the older adult population in post-acute/long-term care preferred.
- Knowledge of geriatric population and dementia.
