About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our team at Lutheran Social Services of South Dakota. As a Care Coordinator, you will play a vital role in supporting youth and families facing challenges by implementing social work case management and facilitating Family Team Conferencing and the Wraparound process.

Responsibilities
  • Implement social work case management to children, youth, and their families
  • Facilitate Family Team Conferencing and the Wraparound process
  • Promote a family driven, youth guided, community based, culturally competent and trauma informed system of care
  • Seek appropriate community resources for the family and monitor the quality of services provided
  • Collect and document required data to ensure the process is outcome-driven and model fidelity is upheld
Requirements
  • Bachelor's Degree in social work or a related field, plus one to two years of related experience
  • Licensed as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners
  • Valid driver's license and ability to obtain approved driver status
