Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Westampton, New Jersey.

Job Description:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team, working closely with nurses, therapists, and other healthcare professionals to deliver comprehensive care to our clients.

Responsibilities:
  • Conduct home visits to assess clients' physical abilities and develop personalized care plans.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance exercises, to improve clients' mobility and function.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Provide education and training to clients and their families on rehabilitative care and activities necessary for independent living.
  • Document observations, interventions, and evaluations in a timely and accurate manner.
Requirements:
  • Current New Jersey Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
