Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Westampton, New Jersey. As a Physical Therapist, you will be responsible for providing high-quality care to adult and geriatric clients in their homes, promoting their independence and well-being.
Responsibilities:- Conduct home visits to assess clients' physical needs and develop personalized care plans.
- Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance assessments, to improve clients' mobility and function.
- Collaborate with the multidisciplinary care team to ensure seamless care coordination and communication.
- Educate clients and their families on rehabilitative care and activities necessary for independent living.
- Accurately document client progress and care plans using electronic medical records.
- A current New Jersey Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
