Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Amber Home Health Care. As a Home Health Care Registered Nurse, you will provide high-quality nursing care to patients in their place of residence, working closely with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Administer skilled nursing care to patients on an intermittent basis, in accordance with physician orders and plan of care.
  • Makes initial evaluation visits and regularly reevaluates patient's nursing needs, initiating necessary revisions to the plan of care.
  • Provides services requiring substantial specialized nursing skills, including preventive and rehabilitative nursing procedures.
  • Prepares clinical and progress notes for each patient visit, as well as summaries of care conferences.
  • Coordinates services, informs personnel of changes in patient condition and needs, and counsels patients and families on meeting nursing and related needs.
  • Participates in and presents inservice programs, understands and adheres to established Agency policies and procedures, and processes orders and notifies physicians of patient needs and changes in condition.
  • Determines the amount and type of nursing needed by each individual patient, refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker as necessary, and supervises and teaches other nursing personnel.
  • Conducts patient care conferences, participates in peer review and Quality Assessment and Performance Improvement, and gives total patient care as needed.
  • Takes on-call duty nights, weekends, and holidays, as assigned, and completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency defined time frames.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • Experience in home health care or a related field, with a focus on skilled nursing care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to prioritize tasks, manage time, and maintain accurate records.
  • Knowledge of ICD-10 codes and OASIS assessments.
