Key Responsibilities- Assessment and Planning: Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Care Coordination: Coordinate and manage patient care, including arranging for equipment and services as needed.
- Communication: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Education and Support: Provide education and support to patients and families on self-care techniques, medication management, and other health-related topics.
- Documentation and Reporting: Maintain accurate and detailed records of patient interactions, care plans, and outcomes.
- Education: Graduation from an accredited school of nursing.
- Experience: 12 years of recent acute care experience in an institutional setting (required). 12 years of recent home care experience (preferred).
- Skills: Strong assessment and planning skills, excellent communication and interpersonal skills, ability to work independently and as part of a team.
- Competitive Compensation: $72,000 - $82,000 per year.

As a Hospice Registered Nurse (RN) Case Manager with Resource 1 Homecare Staffing, you will have the opportunity to work in a dynamic and supportive environment, with a mix of facility and home-based hospice care. You will be assigned a centralized territory and will be responsible for managing an average caseload of 12-15 patients.
