Role Summary

The Oncology Navigator plays a crucial role in enhancing the patient journey by ensuring a smooth navigation process from diagnosis through the continuum of care to survivorship. This position offers personalized support to patients, families, and caregivers, aiming to eliminate barriers to care and facilitate timely access to necessary services.

Key Responsibilities
  1. Facilitate coordination of patient care services to guarantee high-quality patient experiences and efficient patient flow.
  2. Assist patients in accessing care promptly by coordinating and facilitating appointments.
  3. Accompany patients during consultation appointments to provide support and guidance.
  4. Follow patients throughout their journey from diagnosis to survivorship, ensuring continuity of care.
  5. Provide seamless referrals to resources, addressing any barriers to care.
  6. Assess patients' current and future needs, connecting them with internal and external resources, and conducting distress screenings as necessary.
  7. Act as a patient advocate, ensuring their needs and concerns are addressed throughout the treatment process.
  8. Collaborate with clinical teams to develop, implement, and adhere to clinical and research guidelines for optimal patient outcomes.
  9. Engage with research teams to ensure appropriate patient screening for potential participation in clinical trials.
  10. Maintain close collaboration with physicians and the cancer care team to ensure seamless care coordination.
  11. Participate actively in multidisciplinary conferences to discuss patient care strategies.
  12. Provide education to patients regarding their conditions and support for symptom management.
  13. Track navigation program metrics and quality indicators, documenting patient interactions and progress.
  14. Collaborate with leadership to plan, develop, implement, and evaluate new or expanded programs and services.
  15. Engage in community health promotion and awareness initiatives.

Qualifications

Required Education:
Associate's Degree in Nursing

Required Experience:
Minimum of five years of nursing experience

Licensure and Certifications:
Registered Nurse License from the appropriate regulatory body

Essential Skills:
Strong organizational and planning abilities, effective written and verbal communication, time management skills, and proficiency in database management and forms development.
