Job Summary:

We are seeking a skilled Foster Care Coordinator to join our team at UnitedHealthcare. As a Foster Care Coordinator, you will be responsible for providing trauma-informed care to children and youth in foster care, working closely with caregivers and other stakeholders to ensure the best possible outcomes for our members.

Responsibilities:

  • Assess, plan, and implement care strategies that are individualized for children and youth in a family-oriented context, considering reunification and/or other permanency planning needs, needs for adoptive children, and needs for transition-age youth with alternative permanency plans.
  • Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services.
  • Manage the care plan/plan of service throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for children, youth, and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team.
  • Provide trauma-informed and disease/disorder-specific education to members and caregivers.
  • Coordinate with the child welfare and juvenile justice system on behalf of members and caregivers.
  • Coordinate with additional services, including but not limited to, educational/educational advocacy, complex behavioral health (PRTF, etc.), services in line with state Family First Prevention and Services Act for children and families, high fidelity wraparound services, early childhood intervention.

Requirements:

  • Master's degree in social work, nursing, psychology, or related human services area.
  • 2+ years of experience working within the community health setting in a health care role.
  • 1+ years of experience working with multi-system children.
  • 1+ years of experience working with MS Word, Excel, and Outlook.
  • Ability to travel in assigned region to visit Medicaid members in foster homes and/or other settings, including foster care agencies, community centers, hospitals, or providers' offices.
  • Reside locally to Reno, Butler, and Sedgwick Counties, KS.

Preferred Qualifications:

  • Licensed Social Worker or clinical degree.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting.
  • Experience with arranging community resources.
