We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at LHC Group. As a LPN, you will play a vital role in providing quality patient care and performing technical skilled care in the patient's home.

Responsibilities:

  • Provide quality patient care and perform technical skilled care in the patient's home
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Conduct thorough assessments of patients' physical and emotional needs
  • Administer medications and treatments as prescribed by physicians
  • Monitor patients' progress and report any changes or concerns to the supervising nurse and/or physician

Qualifications:

  • Current LPN licensure in the state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • Certification required
