American Family Care (AFC) Urgent Care is on the lookout for skilled Nurse Practitioners and Physician Assistants to enrich our vibrant team. Candidates should possess a minimum of two years of experience in urgent or primary care settings.


Position Summary:

As an integral member of our healthcare team, you will engage in the assessment, diagnosis, and treatment of patients, collaborating closely with a designated physician. Your contributions are vital in delivering exceptional care and fostering positive health outcomes.


Key Responsibilities:
  • Evaluate and manage patients presenting with common acute conditions, illnesses, or minor injuries in partnership with a physician.
  • Collaborate with a physician to oversee the healthcare of patients with chronic conditions.
  • Diagnose and address acute, chronic, and long-term health issues.
  • Provide education to patients and their families regarding preventive care, healthy practices, medical concerns, and the appropriate use of prescribed treatments and medications.
  • Maintain precise, clear, and confidential medical documentation.
  • Record all medical assessments, diagnoses, procedures, treatments, outcomes, educational efforts, and referrals.
  • Conduct Department of Transportation (DOT) physical examinations as a Medical Examiner.
  • Order necessary diagnostic tests and prescribe medications within your professional scope.
  • Facilitate the review of medical records by physicians and peers to ensure quality standards are met, implementing constructive feedback as required.
  • Encourage preventive and health maintenance practices, including annual check-ups, positive health behaviors, and self-care skills through education and counseling.
  • Incorporate new knowledge from professional development opportunities into clinical practice.
  • Regular attendance is crucial for the smooth operation of the clinic.
Additional Responsibilities:
  • Assist in managing patient flow through triage and other nursing tasks as necessary.
  • Perform other assigned duties as needed.
Qualifications:
  • Proficient in Electronic Medical Records (EMR) systems.
  • Flexibility to work weekends and holidays as needed.
  • Self-motivated with a commitment to delivering quality patient care and ensuring satisfaction.
  • Strong communication skills and the ability to collaborate effectively within a team.
  • Willingness to work with various covering physicians in addition to the primary collaborating physician.
  • A positive attitude with a focus on patient-centered care.
Education and Experience:
  • Current licensure as a Certified Registered Family Nurse Practitioner (ANCC or AANP) or as a Physician Assistant (PANCE).
  • At least two years of experience in primary or urgent care, or internal medicine.
