Job Summary

Aveanna Healthcare is seeking a compassionate and skilled LPN Nurse to join our team as a Private Duty Home Health Care Specialist. As a key member of our care team, you will provide high-quality patient care and support to our clients in the comfort of their own homes.

Responsibilities
  • Implement and manage patient care plans in accordance with physician orders and company policies.
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document patient care and progress in a timely and accurate manner.
  • Provide education and training to patients and their families on health promotion and disease management.
  • Collaborate with the care team to ensure seamless transitions of care and address any concerns or issues that may arise.
Requirements
  • Active, unencumbered LPN license in the state of practice.
  • Current CPR certification from a recognized organization.
  • TB skin test (current within the last 12 months).
  • At least six months of prior hands-on nursing experience (preferred but not required).
  • Reliable transportation.
