Job Summary

Key Responsibilities
  • Program Leadership
    • Provide guidance, oversight, and supervision to licensed behavioral health providers, ensuring they meet program requirements and provide high-quality patient care.
    • Lead the development and implementation of program policies, procedures, and quality improvement initiatives.
    • Collaborate with the Medical Director and Program Coordinator to ensure the program's strategic goals and objectives are met.
  • Clinical Services
    • Conduct behavioral health screenings and assessments, developing individualized care plans and recommending appropriate levels of care.
    • Provide counseling, recovery support, relapse prevention, life skills training, and psycho-education to patients in individual, group, and family settings.
    • Coordinate patient referrals to mental health, substance use disorder, and respite care providers, programs, and facilities.
  • Concrete Services
    • Assess and address socio-economic needs of patients, promoting access to supportive housing, public entitlements, employment, and other community resources.
    • Collaborate with hospital and ER staff for discharge planning of homeless patients.
    • Partner with local community health and human service providers to promote access to care and enrollment into the program.
  • Quality Improvement
    • Participate in the development and implementation of quality improvement initiatives, ensuring the program meets clinical performance outcomes.
    • Collaborate with the QI committee to evaluate progress and devise corrective actions to support progress toward goals.
  • Grant Writing and Reporting
    • Assist with grant writing and reporting for behavioral health initiatives, in conjunction with the Program Coordinator.
Qualifications
  • Education
    • Master's degree in behavioral health from an accredited program.
  • Experience
    • Minimum of two years' experience in behavioral health, outpatient, or community health settings.
  • Licensure/Certification
    • LCMHC/LICSW in the State of New Hampshire and MLADC.
