Key Responsibilities:

  • Support the development of staff schedules and agendas
  • Manage incoming communications for the Hospice Team
  • Assist in managing referrals by collecting necessary information, updating referral logs, and collaborating with the Nursing Director
  • Facilitate the ordering of Durable Medical Equipment
  • Work in partnership with hospice team members to ensure compliance with patient documentation requirements
  • Prepare new admission paperwork and supplies for Hospice Nurses
  • Organize information for Interdisciplinary Team meetings
  • Complete and route referral/intake forms for admission approval
  • Adapt to a fast-paced environment with flexibility
  • Perform additional duties as assigned

Qualifications:

  • Current LPN License
  • Preferred experience in a Hospice environment
  • Quality Assurance experience is a plus
  • Proficient in computer applications, including Excel and Word
  • Strong service orientation
  • Enthusiasm for joining a growing team
