Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our Hospice Team in Akron. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care, developing and implementing individualized care plans, and ensuring seamless transitions between care settings.

Responsibilities
  • Administer skilled nursing care to patients of all ages in their place of residence
  • Coordinate care with the interdisciplinary team, patients, and their families
  • Develop and implement individualized care plans to meet patient needs
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Provide education and support to patients and their families
  • Document patient information and care plans in a timely and accurate manner
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • One year of experience as a Registered Nurse in a clinical care setting
  • Home health experience preferred
