POSITION SUMMARY:
This role involves evaluating and coordinating services related to the social, emotional, and financial requirements of Care Management participants. It includes planning care to address these needs and assessing eligibility for entitlement programs and essential services. The implementation of long-term care service plans is also a key responsibility.

KEY RESPONSIBILITIES:
Conduct thorough assessments of client health requirements.


Utilizing a person-centered approach, collaborate with the participant, their support network, and their physician to develop a tailored care plan.

Assist participants in determining the frequency and duration of services, and execute the plan as approved by the participant.


Provide advocacy and facilitate arrangements for care that may not be available through formal networks (e.g., socialization, service needs) as outlined in participant care plans.

Monitor participant conditions and the delivery of services in relation to care plans. Adjust plans as necessary based on reassessments or ongoing monitoring. Maintain open communication with the participant's family and physician.

Keep detailed participant files and a tracking system for those applying for and receiving entitlement services, along with other relevant program documentation.

Seek clinical guidance from supervisory staff as required to ensure high-quality care plans.

Manage the eligibility process for financial assistance for participants, including liaising with DHHS Eligibility Specialists and preparing necessary applications.

Stay informed on current trends in gerontological social work and the skills necessary for success.

Adapt to evolving guidelines related to the MI Choice Waiver contract, among others.

QUALIFICATIONS:

  • Valid Michigan driver's license is required.
  • Reliable transportation and current proof of automobile insurance are necessary.
  • Must hold a current Michigan Licensed Social Worker license.
  • Annual tuberculosis testing/review is required.
  • Must successfully pass all background checks.
EDUCATION and/or

EXPERIENCE:
A Bachelor's degree in social work from an accredited institution, with two to three years of relevant experience. A current State of Michigan Social Worker license (LLBSW or BSW) is mandatory. A minimum of one year of experience in home care is preferred, along with experience working with elderly and/or disabled adults.
