About the Role

This is a challenging and rewarding role that requires a high level of expertise in patient care and coordination. As a Kidney Transplant Coordinator RN, you will be responsible for delivering and demonstrating strong clinical experiences in patient care, accurately documenting direct, age-specific care to patients or groups of patients based on medical orders and an integrated, multidisciplinary plan of care.

Key Responsibilities
  • Coordinate and care for complex patients, families, or communities to actual or potential health problems.
  • Assist the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort.
  • Provide direction, education, and leadership to the healthcare team.
  • Deliver and demonstrate strong clinical experiences in patient care, accurately documenting direct, age-specific care to patients or groups of patients based on medical orders and an integrated, multidisciplinary plan of care.
Requirements
  • Active Virginia RN license.
  • BLS, ACLS certification.
  • Experience within a surgical department preferred, and a strong history of patient navigation and coordination of care for surgical patients.
  • Prolonged standing and walking in the hospital and OR setting, as well as holding clinics with patients in an outpatient setting.
  • Able to lift, position or transfer patients. Able to lift supplies and equipment.
