  • Engage with parents to ensure their active participation in their child's treatment for better clinical outcomes.

  • Assist families in achieving their goals as outlined in Wellness Plans, regularly reviewing and updating these plans as necessary.
  • Support families in navigating and accessing necessary services and resources to help them reach their objectives.
  • Maintain communication with families during hospitalizations, participating in discharge planning and collaborating with all relevant providers to create aftercare plans.
  • Offer educational resources and skill-building opportunities to families on topics such as daily living skills, coping strategies, and symptom management.
  • Develop and implement crisis intervention plans with families, providing necessary support during and after standard working hours.
  • Help families establish natural support systems by collaborating with family members, educational staff, and legal guardians.
  • Work closely with interagency team members to advocate for the mental health needs of children and families.
  • Ensure timely and accurate documentation in collaboration with clients and families, adhering to agency policies.
  • Meet service provision expectations based on client needs, delivering services in home settings, including evenings and weekends.
  • Other duties as assigned.

    Integrated Health Team Collaboration:
  • Engage with other members of the integrated health team, including psychiatrists, therapists, and primary care providers, to discuss all aspects of client wellness.
  • Incorporate feedback from integrated health team providers into clients' Wellness Plans.
  • Make referrals to additional services as necessary to support clients' integrated health needs.
  • Act as a liaison among clinical providers, families, schools, and other stakeholders involved in the client's treatment.

    Additional Information:
  • Seek culturally competent services to meet the diverse needs of each client and family.
  • A valid driver's license and minimum automobile coverage are required for this position.
  • Qualifications: High School diploma or GED is required; higher education in a human service-related field is preferred. Relevant experience is required based on educational background.
  • All experience must involve working with individuals facing psychiatric, substance use, or developmental challenges.
